Been digging using transillumination during exams


I have intra oral cameras in my 4 ops.  2 of them have digidoc cameras and at midwinter I splurged on a trans illuminator for one to see if it would help me show patients what a crack can look like or to see decay that may not be easy to see with BW.  This was a case from today that the pt will be doing 2 crowns after seeing the photos.  I tell people all the time to take lots of photos to explain to patients what is going on in their mouth.  It allows them to see it and take responsibility for it.  This is from today.  I love investing in technology that helps us communicate with our patients.  they say a picture is worth 1000 words, but it this case it was the pt realizing she needed 2 crowns to protect those cracked teeth.

Very nice.  I just got a microlux Illuminator it was cheap and does a great job like you showed.  How did you get your pictures?  They are great to show patients.

So the microlux trans illuminator works with your IOC to get the camera shots? I remember being demoed one with the camera built in. Can't remember  what company it was....

Did you get the attachment for the digitaldoc camera?


Rich - how much did this set you back if you remember roughly?

 

And this is digi-doc camera with Lum added on correct?


Great pics Rich. Buying th me digidoc had cameras are a hard pill to swallow at $11k for a pair. But the mouthwatch are showing exactly what I want so far. What trans illuminate is this and is there a camera as part of it? Thanks brother

Is this something we saw at IDS several years ago?  They wouldn't let me buy one at that time.


So this is an attachment for the digidoc called the LUM.  Jeff, you are correct when you talk about the cost of their cameras.  they are crazy expensive.  I have 2 iris cameras and they were prob 5k or so.  the lum was another 2700.  That being said, this case just bought that LUM.  This slips over the iris camera and has a rubber type attachment light is illuminated through.  you place the rubber attachment over the tooth and just click to snap the pic.  

 

Mike, I'll look for them.  I have some old pics of the cracked teeth somewhere in her chart.  we have been talking about this for 3 years and this pic resonated with her for some reason to schedule.

You couldn't have just fixed that with some duct tape and Windex???


On a more serious note, those pics look similar to what you would see using the CariVu, by Dexis.  And sounds like it's much less expensive!

We have a digidoc but I don't use it much. Use the shick cam. The reason is that the drivers have to be continually reinstalled in eaglesoft for some reason. Just got tired of having to do it so much. Anyone have this problem or a solution?

 


Great pics Rich.... have you had these submitted as evidence of preop incomplete fracture for insurance preauths?

I'm lucky (or unlucky) in that my digidoc rep is a pt.  I just give him a call and he takes care of stuff if there is an issue.  give you rep a call and have them work it out.

mike, here is a photo of both teeth but further away.  this is what we had.  Its not like you can't tell, but I think all the cracks when trans illumination is done makes it jump out a bit more because you see the internal

 

and here is the failing sealant.  pt sees the stain but the shadow on the trans illumination in here case got her to accept it

Mark, I have not had to yet, but when I have a pt that I think has cracked tooth syndrome, or I have a tooth that just has lots of apparent cracks like these did, I like to take these shots because they really detail how deep some of them go.  This way if we do get denied, its just more info I have to fight things 


Damn...just sold the diagnodents and got Carivue...with my luck, it wont even work :(

I just bought the CariesVue and have Eaglesoft, there is a bridge.  We have training next week, so haven't really tried it out yet, except on an assistant, seems the bridge works well.  We only got one camera to share around the office, have Intraoral cameras in each room.

I'm sure 100% of us on here agree with how awesome this is and that treatment on this would be justified.  But the second they see a shallow to mid sized amalgam doesn't this become declined and possibly even the version where we can even charge the patient?

i hate to bring it up,but this is the reality many of us live in... is absolutely insane the mentality of "we don't cover until its broke BROKE", but that is our current reality?  bear in mind probably ZERO DDS or staff on deltas employee list lives or acts this way... who waits for their tires to blow before changing their tires?  at 3/16 we all change our tires...  but i guess tires ARE more important than teeth...

Steve, I'm majority PPO.  prob 30% FFS.  I take these for 2 reasons.  They help the pt see the problem and it is a good starting pic to send to the insurance.  I then take photos when I open the tooth up.  I will leave decay and such in there to make sure the ins co sees what I'm seeing.  Very rarely to we get pushback or denials.  Document, Document, Document!


Been wanting to pull the trigger on Carivu for a little while, excited to see others using it and getting great results!
